Patients will be eligible for the study if ALL the following are present:

Inclusion criteria:

  • Age ≥18 years; AND
  • Suspected community-acquired Acute Respiratory Infection; AND
  • Intended hospitalisation; AND
  • No medical history might, in the opinion of the attending clinician, put the patient at significant risk if he/she were to participate in the trial.

 

Exclusion criteria:

Patients will be excluded if the treating clinician considers that the patient is not suitable for the trial. 
Patients may be excluded if any ONE of the following are present:

  • Past, current or planned participation in other research studies within 30 days (e.g. during COVID waves or influenza season); 
  • Vulnerable subjects (pregnancy; cognitively impaired; prisoners; students; employee; minorities); 
  • Cardiac arrest or post-cardiac arrest return of spontaneous circulation;
  • Not expected to survive 3 days due to pre-existing chronic disease; 
  • Palliative (comfort) care
  • Undergoing active cancer therapy;
  • Neutropenia due to chemotherapy/malignancy (but not due to sepsis)
  • Discharged from hospital within the previous 28 days;
  • Immunocompromised or being treated with immunotherapy
  • Organ transplantation
  • HIV and on HIV drugs (indinavir, atazanavir, nelfinavir, saquinavir, ritonavir)
  • Recent Surgery (within one month)
  • Dialysis (including Continuous Ambulatory Peritoneal Dialysis)
  • Diabetic ketoacidosis
  • Acute asthma
  • Recurrent chest infection, 
  • Cushing’s or Addison’s disease, 
  • Long term systemic steroid 
  • Long-term antibiotics
  • HbA1C level ≥6.5% on the day of ED admission

誰可以加入研究?

如果滿足以下所有條件，患者將有資格參加該研究：

納入標準:

  • 18歲或以上和
  • 疑似社區型急性呼吸道感染；和
  • 住院治療；和
  • 主治臨床醫師認為，如果患者參加試驗，沒有任何病史可能會使患者面臨重大風險。

 

排除標準:

如果臨床醫師認為患者不適合參加試驗，患者將被排除。

如果存在以下任何一種情況，患者可能會被排除：

  • 過去、目前或計劃在 30 天內參與其他研究（例如在新冠肺炎浪潮或流感季節期間)；

  • 弱勢族群（懷孕、認知障礙、受刑人、學生、員工、少數族群);

  • 心臟驟停或心臟驟停後自發性血液循環之回復；

  • 由於已有慢性疾病，預計無法存活 3 天；

  • 安寧（舒適）護理;

  • 正在接受積極的癌症治療；

  • 化療/惡性腫瘤引起的中性粒細胞減少症（但不是敗血症引起的);

  • 過去 28 天內出院；

  • 免疫功能低下或正在接受免疫治療;

  • 器官移植;

  • 人類免疫力缺乏病毒與人類免疫力缺乏病毒藥物 (indinavir, atazanavir, nelfinavir, saquinavir, ritonavir);

  • 最近的手術（一個月內);

  • 透析（包括連續性可攜帶腹膜透析);

  • 糖尿病酮酸中毒;

  • 急性哮喘;

  • 反覆呼吸系統感染;

  • 庫欣氏症或愛迪生氏病;

  • 長期服用全身性類固醇;

  • 長期服用抗生素;

  • 急診入院當天 HbA1C 水平 ≥6.5%
