Antibiotic Stewardship in Suspected Neutropenic Fever (ASTERIC): A multi-centre, type 1 hybrid effectiveness-implementation, stepped-wedge, randomized controlled trial.

 

 

 

 

Research purpose:

To evaluate the effectiveness of antibiotic stewardship intervention for patients with suspected neutropenic fever (sNF) in Hong Kong’s emergency departments (EDs), aiming to reduce unnecessary antibiotic use and improve patient outcomes.

 

Main question: 

In adult patients with suspected neutropenic fever attending emergency departments and planned hospital admission, does the ASTERIC protocol reduce unnecessary antibiotic use and the risk of serious adverse events compared to usual care within 30 days from randomization?

 

Key facts:

  • Setting: The trial will take place in the EDs or wards of 8 hospitals across Hong Kong and will operate 24/7.
  • Participants: The trial aims to recruit 416 adult patients (208 in each group).
  • Interventions:
    • Intervention: Usual Care + ASTERIC protocol.
    • Control: Usual Care + Neutropenic Fever Protocol.

Duration:

1st January 2025 – 31st December 2026 (24 months).

ASTERIC 試驗
疑似嗜中性白血球低下症合併發燒的抗生素管理 (ASTERIC)：多中心、有效性-實施混合一型、階梯式楔形、隨機對照試驗。


 



 

研究目的:

評估抗生素管理干預措施在香港急症室中對疑似嗜中性白血球低下症合併發燒 (sNF) 患者的效能，目的是減少不必要的抗生素使用並改善患者的治療效果。

 

主要問題:

在急症室就診、並計劃住院的疑似嗜中性白血球低下症合併發燒的成人患者中，與常規護理相比，ASTERIC 協議是否能在隨機分組後 30 天內減少不必要的抗生素使用並降低嚴重不良事件的風險？

 

關鍵事實:
  • 研究場所：該試驗將在香港 8 家醫院的急症室或病房進行，並將全天候運作。
  • 參與者：試驗計劃招募 416 名成人患者（每組 208 名）。
  • 干預措施：
    • 干預組：常規護理 + ASTERIC 協議。
    • 對照組：常規護理 + 嗜中性白血球低下症合併發燒協議。
研究期限:

2025 年 1 月 1 日至 2026 年 12 月 31 日（24 個月）。
